VirtualHealthCheck.com -- A chronic disease management program focusing on improved patient care and the reduction of ambulatory expenses among chronic disease patients. This new level of care will utilize an existing telemedicine approach while combining a novel, Web-based monitoring and management software with a service (SaaS) platform, VirtualHealthCheck.com.

Improved Patient Care While Reducing Ambulatory Expenses

The program goal is to provide Internet-based patient-to-practitioner chronic care patient monitoring. The foundation of this program is the understanding that early physician intervention increases care quality and reduces hospitalizations and health care costs. Unlike most current disease management programs, physicians control this process. They design the patient protocols, review the results and intervene where appropriate.

In-home Monitoring

The patient monitoring consists of in-home blood pressure, weight, and blood sugar vitals and a regularly scheduled patient call. Patient vital signs are transmitted over a wireless Bluetooth connection to a Netbook PC which transmits data to the wellness contact center. The patient’s vital signs, medical records and the contact information are merged into a secure web application for physician review.

A wireless Bluetooth blood pressure monitor, glucose monitor and/or scale transmit patient data to a Netbook device that stores and send data via a secure https: connection to the vital sign database. This data will be accessible via a Web application at the clinic where a doctor can review it, assign new protocols and have the ability to reach out to the patient via their channel of choice (e-mail, phone, text message, Web video). Data on each patient interaction is captured and stored to provide visibility and a clear compliance trail.

The Patient Wellness Survey

On a scheduled basis, patients are surveyed with questions designed by the physician. The patient survey is a “smart design” that prompts additional questions based on patient responses. A PRM (Patient Relationship Management) application collects the data and populates the physician’s Web application. All calls, texts, emails are recorded, indexed and archived providing a complete compliance audit trail.

Secure Physician Portal

The heart of the system is a Web application that merges patient vital signs and information from all patient interactions. Physicians can create, store and edit patient protocols and surveys within the VirtualHealthCheck platform. By setting parameters specific to vital sign readings, VHC can effectively flag patients who are violating their protocols.Any vital statistic measured that violates an established protocol can lead to immediate patient interviews where a health care professional can determine what circumstances are adversely affecting patient health. It is this real-time monitoring of patient vitals along with proactive patient interviewing that helps solve problems before they escalate into expensive patient hospitalizations
